Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to revoke Dr. Hu-Nam Nam's authorization to treat injured workers within the New York State workers' compensation system effective June 23, 2014. Dr. Nam's last known business address is 172-13 Hillside Avenue, Suite 200, Jamaica, New York.

The revocation is based upon Consent Order (BPMC No. 13-142), issued by the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective December 18, 2013.

Dr. Nam could not successfully defend against at least one of the charged acts of misconduct alleging ordering excessive testing and treatment. Her license was suspended for one month and she is on probation for 36 months. Dr. Nam's license is also limited, precluding her, individually or through a professional corporation, from treating or billing patients through No-Fault insurance or Workers' Compensation. She also cannot own more than one professional corporation at any time and may only own a single professional practice entity if her medical practice is performed at that site.

Dr. Nam is prohibited from rendering treatment and care to injured workers. Reports submitted by Dr. Nam for services rendered prior to June 23, 2014, are valid, but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Nam for services rendered prior to June 23, 2014 should not be denied due to the revocation of her authorization.
