GANGADHAR MADUPU, M.D. (Authorization No. 216519-9W)

Please be advised that, effective May 27, 2010, the New York State Department of Health, State Board for Professional Medical Conduct, by Surrender Order (BPMC No. 10-83), ordered the surrender of the medical license of Gangadhar Madupu, M.D. No current business address is available for Dr. Madupu.

Dr. Madupu is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Madupu for treatment rendered prior to May 27, 2010, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Madupu regarding treatment he rendered prior to May 27, 2010, should not be denied due to the surrender of his license to practice medicine.

ILYA MARK SMUGLIN, M.D. (Authorization No. 221943-4W)

Please be advised that, effective May 24, 2010, the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), issued a Nondisciplinary Order of Conditions pursuant to New York Public Health Law Section 230 precluding Ilya Mark Smuglin, M.D., either individually or through a professional corporation, from evaluating, treating or billing patients through either no-fault insurance or workers compensation. This Order shall remain in effect until and unless modified by a subsequent Order of the BPMC. No current business address is available for Dr. Smuglin.

Dr. Smuglin is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Smuglin for treatment rendered prior to May 24, 2010, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Smuglin regarding treatment he rendered prior to May 24, 2010, should not be denied due to the condition of preclusion imposed on his license to practice medicine.
