JOSEPH D. BEASLEY, M.D. (Authorization No. 144938-8W)

Please be advised that, effective February 1, 2010, the New York State Department of Health, Board for Professional Medical Conduct, by Modification Order (BPMC No. 99-182), adopted the agreement of Joseph D. Beasley, M.D., to never reactivate his registration or reapply for a license to practice medicine in the State of New York. The modification of Consent Order BPMC No. 99-182 does not constitute a new disciplinary action. No current business address is available for Dr. Beasley.

Dr. Beasley is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Beasley for treatment rendered prior to February 1, 2010, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Beasley regarding treatment he rendered prior to February 1, 2010, should not be denied due to the permanent inactive status of his license to practice medicine.

JOHN ANDREW CAROTHERS, M.D. (Authorization No. 108308-8W)

Please be advised that, effective February 8, 2010, a Hearing Committee of the New York State Department of Health, Board for Professional Medical Conduct, issued a Determination and Order (BPMC No. 10-16), by which the license to practice medicine of John Andrew Carothers, M.D., is indefinitely suspended. No current business address is available for Dr. Carothers.

Dr. Carothers is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Carothers for treatment rendered prior to February 8, 2010, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Carothers regarding treatment he rendered prior to February 8, 2010, should not be denied due to the indefinite suspension of his license to practice medicine.

STEVEN HOWARD CHARNO, M.D. (Authorization No. 116271-8W)

Please be advised that, effective February 1, 2010, the New York State Department of Health, Board for Professional Medical Conduct, by Surrender Order (BPMC No. 10-09), adopted the permanent surrender of Steven Howard Charno, M.D.'s license to practice medicine in the State of New York. No current business address is available for Dr. Charno.

Dr. Charno is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Charno for treatment rendered prior to February 1, 2010, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Charno regarding treatment he rendered prior to February 1, 2010, should not be denied due to the permanent surrender of his license to practice medicine.

SAJI FRANCIS, M.D. (Authorization No. 207476-3B)

Please be advised that, effective February 15, 2010, the New York State Department of Health, Board for Professional Medical Conduct, issued a Non-disciplinary Order of Conditions, by which Saji Francis, M.D., is precluded from all practice of medicine in New York State or any jurisdiction where the practice of medicine is based solely upon a New York license until final disposition of the current investigation. No current business address is available for Dr. Francis.

Dr. Francis is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Francis for treatment rendered or examinations conducted prior to February 15, 2010, are valid, but are invalid for any treatment rendered or examinations conducted on or after that date until further notice. Requests to cross-examine Dr. Francis regarding treatment he rendered or examinations he conducted prior to February 15, 2010, should not be denied due to his indefinite preclusion from the practice of medicine.

VANESA GREGORY, D.O. (Authorization No. 212965-8W)

Please be advised that, effective March 8, 2010, a hearing Committee of the New York State Department of Health, Board for Professional Medical Conduct, issued a Determination and Order (BPMC No. 10-29), whereby Vanesa Gregory, D.O.'s license to practice medicine in New York State was revoked. Previously, by Non-practice Order, Dr. Gregory was precluded from the practice of medicine, effective September 22, 2006, until final disposition of the then investigation. No current business address is available for Dr. Gregory.

Dr. Gregory is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Gregory for treatment rendered prior to September 22, 2006, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Gregory regarding treatment she rendered prior to September 22, 2006, should not be denied due to her preclusion from the practice of medicine and revocation of her license to practice medicine.

DAVID W. O'CONNELL, M.D. (Authorization No. 138181-3W)

Please be advised that, effective February 23, 2010, the New York State Department of Health, Board for Professional Medical Conduct, by Modification Order (BPMC No. 04-33), adopted the agreement of David W. O'Connell, M.D., to never reactivate his registration or reapply for a license to practice medicine in the State of New York. The modification of Consent Order BPMC No. 04-33 does not constitute a new disciplinary action. No current business address is available for Dr. O'Connell.

Dr. O'Connell is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. O'Connell for treatment rendered prior to February 23, 2010, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. O'Connell regarding treatment he rendered prior to February 23, 2010, should not be denied due to the permanent inactive status of his license to practice medicine.

ALEXANDER ROZENBERG, M.D. (Authorization No. 207165-2W)

Please be advised that, effective March 8, 2010, the Commissioner of the New York State Department of Health, through the State Board for Professional Medical Conduct, issued a Commissioner's Order and Notice of Referral Proceeding whereby Alexander Rozenberg, M.D., may not practice medicine in New York State or in any jurisdiction where the practice of medicine is based solely upon a New York license. This Order remains in effect unless modified by the New York State Commissioner of Health. Previously, on March 14, 2007, Dr. Rozenberg agreed to refrain from treating individuals who have suffered work-related injuries and illnesses. No current business address is available for Dr. Rozenberg.

Dr. Rozenberg is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Rozenberg for treatment rendered prior to March 14, 2007, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Rozenberg regarding treatment he rendered prior to March 14, 2007, should not be denied due to his agreement with the Workers' Compensation Board or his indefinite preclusion from the practice of medicine.
