Subject Number 046-394 Permanent Limitation of Medical License of Dr. John Patrick DeSimone

Subject Numbers Regarding Health Provider Authorizations

January 11, 2010

Authorization No. 104285-2W

Please be advised that, by Consent Order (BPMC No. 09-230), the New York State Department of Health, State Board for Professional Medical Conduct, effective January 5, 2010, permanently limited the medical license of John Patrick DeSimone, M.D., to preclude him from all patient contact and any practice of medicine, clinical or otherwise. Previously, Dr. DeSimone's medical license was deemed inactive effective October 31, 2009, by the New York State Education Department. No current business address is available for Dr. DeSimone.

Effective October 31, 2009, Dr. DeSimone is precluded from rendering medical treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. DeSimone for treatment or examinations rendered prior to October 31, 2009, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. DeSimone regarding treatment or examinations he rendered prior to October 31, 2009, should not be denied due to the inactive status of his authorization or the permanent limitation on his license.

Any questions regarding this matter should be referred to the Office of General Counsel at (518) 486-9564.
