Please be advised that, effective December 1, 2009, Glenn Kelly, D.C., voluntarily resigned his authorization to treat individuals who have suffered work-related injuries or illnesses. The voluntary resignation is due to Dr. Kelly's relocation to the State of Virginia and is not disciplinary in nature. No current business address is available for Dr. Kelly.

Dr. Kelly is prohibited from rendering chiropractic treatment and other care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Kelly for treatment rendered prior to December 1, 2009, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Kelly regarding treatment he rendered prior to December 1, 2009, should not be denied due to the resignation of his authorization.
