Please be advised that, effective November 20, 2009, the New York State Department of Health, State Board for Professional Medical Conduct issued an Order for Non Practice of Medicine regarding the medical license of Richard Jay Lewis, M.D. This action is not disciplinary in nature. The Order remains in effect pending final disposition of the current investigation being conducted by the Office of Professional Medical Conduct. No current business address is available for Dr. Lewis.

Effective November 20, 2009, and until further notice, Dr. Lewis is precluded from rendering medical treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Lewis for treatment or examinations rendered prior to November 20, 2009, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Lewis regarding treatment or examinations he rendered prior to November 20, 2009, should not be denied due to the non practice of medicine order regarding his license to practice medicine.
