Subject Numbers Regarding Health Provider Authorizations

HOWARD M. BERKOWITZ, M.D. (Authorization No. 124679-2W)

Please be advised that, effective September 28, 2009, the New York State Department of Health, Board of Professional Medical Conduct, by Surrender Order (BPMC No. 09-178), adopted the surrender of Howard M. Berkowitz, M.D.'s license to practice medicine in the State of New York. Previously, the authorization of Dr. Berkowitz in the New York State workers' compensation system was deemed inactive on September 11, 2004. Later his medical license was rendered inactive by the State Education Department on November 30, 2007. No current business address is available for Dr. Berkowitz.

Dr. Berkowitz is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Berkowitz for treatment rendered prior to September 11, 2004, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Berkowitz regarding treatment he rendered prior to September 11, 2004, should not be denied due to the surrender of his license to practice medicine.

SERGE DELALEU, M.D. (Authorization No. 123061-4W)

Please be advised that, effective September 16, 2009, Serge Delaleu, M.D., permanently surrendered his license to practice medicine in the State of New York pursuant to New York State Public Health Law Section 230.13. No current business address is available for Dr. Delaleu.

Dr. Delaleu is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Delaleu for treatment rendered prior to September 16, 2009, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Delaleu regarding treatment he rendered prior to September 16, 2009, should not be denied due to the surrender of his license to practice medicine.
