MANJULA M. JAY, M.D. aka MANJULA MUDDAPPA, M.D. (Authorization Nos. 208320-2B and 208320-2I)

Please be advised that, by Consent Order (BPMC No. 09-136), of the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective July 29, 2009, Manjula M. Jay, M.D., admitted guilt to the charge of failure to maintain records.

Pursuant to Workers' Compensation Law Section 13-d (5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Jay from the list of authorized physicians, effective September 22, 2009. No current business address is available for Dr. Jay.

Previously, Dr. Jay's authorization to treat was rendered inactive, effective July 27, 2009, for her failure to register a New York State address with the New York State Department of Education. Effective July 27, 2009, Dr. Jay's authorization was limited to independent medical examinations and she was issued authorization number 208320-2I.

Dr. Jay is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Jay for treatment rendered prior to July 27, 2009, and examinations conducted prior to September 22, 2009, are valid, but are invalid for any treatment rendered or examinations conducted on or after these respective dates. Requests to cross-examine Dr. Jay regarding treatment she rendered prior to July 27, 2009, or independent examinations she conducted prior to September 22, 2009, should not be denied due to the inactive status of her authorization or her removal from the list of authorized physicians.

BRUCE P. KLEIN, M.D. (Authorization No. 188806-4W)

Please be advised that, by Consent Order (BPMC 09-140), of the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective July 31, 2009, Bruce P. Klein, M.D., admitted guilt to the charge of negligence on more than one occasion.

Pursuant to Workers' Compensation Law Section 13-d (5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Klein from the list of authorized physicians, effective September 22, 2009. Dr. Klein's business address is 1160 Corporate Drive, Suite 400, Farmington, New York 14425.

Dr. Klein is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Klein for treatment rendered prior to September 22, 2009, are valid, but are invalid for any treatment on or after that date. Requests to cross-examine Dr. Klein regarding treatment he rendered prior to September 22, 2009, should not be denied due to his removal from the list of authorized physicians.
