Please be advised that, effective July 1, 2009, the New York State Department of Health, Board for Professional Medical Conduct adopted a nondisciplinary Order of Conditions pursuant to New York Public Health Law Section 230, which order is for three years and restricts Dr. Mandelblat during the term of the order from treating or billing patients who are reimbursed either through no-fault insurance or workers' compensation. Her last known business address is 585 Schenectady Avenue, Brooklyn, New York 11203.

Dr. Mandelblat is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Mandelblat for treatment rendered prior to July 1, 2009, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Mandelblat regarding treatment she rendered prior to July 1, 2009, should not be denied due to the temporary limitation on her medical license.
