Please be advised that, effective February 12, 2009, Gerardo Yanayaco's authorization to treat workers' compensation claimants was temporarily suspended for one year pursuant to Workers' Compensation Law Section 13-d [4]. Effective September 30, 2008, by Summary Order of the State Commissioner of Health, Dr. Yanayaco was prohibited from the practice of medicine in the State of New York or in any jurisdiction where the practice of medicine is predicated on a valid New York State medical license. Dr. Yanayaco's last known address is 40-37 81st Street, Elmhurst, New York 11373.
