TAKSIN RATNARATHORN, M.D. (Authorization No. 111533–6W)

Please be advised that, by Consent Order (BPMC No. 08-39), effective March 18, 2008, the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), placed Dr. Ratnarathorn on probation for three years, during which he must be monitored by a physician board certified in an appropriate specialty.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Ratnarathorn from the list of authorized physicians, effective June 30, 2008. Dr. Ratnarathorn's current address is 33 Uxbridge Street, Staten Island, New York 10314.

Dr. Ratnarathorn is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Ratnarathorn for treatment rendered prior to June 30, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Ratnarathorn regarding treatment he rendered prior to June 30, 2008, should not be denied due to the restriction of his medical license or his removal from the list of authorized physicians.

FRANK A. LIZZI, M.D. (Authorization No. 096785–1W)

Please be advised that, by Consent Order (BPMC No. 08-98), of the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective June 1, 2008, Dr. Frank A. Lizzi's license to practice medicine in New York State was limited precluding all patient contact and any practice of medicine clinically or otherwise.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Lizzi from the list of authorized physicians, effective June 30, 2008. Dr. Lizzi's current address is 240 New Scotland Avenue, Albany, New York 12208.

Dr. Lizzi is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Lizzi for treatment rendered prior to June 1, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Lizzi regarding treatment he rendered prior to June 1, 2008, should not be denied due to the restriction of his medical license or his removal from the list of authorized physicians.

RICHARD A. MORGAN, D.O. (Authorization No. 221585–3B)

Please be advised that, by Surrender Order (BPMC No. 08-81), the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective June 6, 2008, adopted Dr. Morgan's application for surrender of his license to practice medicine. He was placed on probation precluding him from prescribing any controlled substances from June 6, 2008 and until July 31, 2008, and agreed to surrender his license to practice medicine in the State of New York on July 31, 2008.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Morgan from the list of authorized physicians, effective June 18, 2008. Dr. Morgan current address is P.O. Box 770171, Ocala, Florida 34477-0171.

Dr. Morgan is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Morgan for treatment rendered or examinations conducted prior to June 18, 2008, are valid, but are invalid for any treatment rendered or examinations conducted on or after that date. Requests to cross-examine Dr. Morgan regarding treatment he rendered or examinations he conducted prior to June 18, 2008, should not be denied due to the surrender of his medical license or his removal from the list of authorized physicians. In addition, testimony and/or depositions concerning independent medical examinations conducted through June 18, 2008, may be provided.

YEFIM ISAAC SOSONKIN D.O. (Authorization No. 209924–0W)

Please be advised that, by Determination and Order (BPMC No. 08-86), effective June 10, 2008, a Hearing Committee of the New York State Department of Health, State Board for Professional Medical Conduct (BPMC) sustained the charge finding Dr. Sosonkin was convicted of a felony crime in New York, and voted to revoke his license to practice medicine in New York State.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Sosonkin from the list of authorized physicians, effective June 30, 2008. Dr. Sosonkin's current address is P.O. Box 297154, Brooklyn, New York 11229.

Dr. Sosonkin is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Sosonkin for treatment rendered prior to June 10, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Sosonkin regarding treatment he rendered prior to June 10, 2008, should not be denied due to the revocation of his medical license or his removal from the list of authorized physicians.

WILLIAM JOHN ZEHNER, JR., M.D. (Authorization No. 181937–4W)

Please be advised that, by Consent Order (BPMC No. 08-92), effective June 19, 2008, the New York State Department of Health, State Board for Professional Medical Conduct (BPMC) imposed license suspension for forty-eight months, stayed with probation for forty-eight months, which includes a practice monitor and practice supervisor.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Zehner from the list of authorized physicians, effective July 2, 2008. Dr. Zehner's current address is 736 Irving Avenue, Syracuse, New York 13210.

Dr. Zehner is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Zehner for treatment rendered prior to July 2, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Zehner regarding treatment he rendered prior to July 2, 2008, should not be denied due to the restriction of his medical license or his removal from the list of authorized physicians.

IGOR ROSIEN, M.D. (Authorization No. 209198–1W)

Please be advised that, by Consent Order (BPMC No. 08-100), effective June 23, 2008, the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), adopted Dr. Igor Rosien's agreement not to contest the charge of negligence on more than one occasion and not to practice emergency medicine until he has completed a clinical competency assessment in emergency medicine.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Rosien from the list of authorized physicians, effective July 7, 2008. Dr. Rosien's current address is 156 West Avenue, Brockport, New York 14420.

Dr. Rosien is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Rosien for treatment rendered prior to June 23, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Rosien regarding treatment he rendered prior to June 23, 2008, should not be denied due to the restriction of his medical license or his removal from the list of authorized physicians.

JOSEFINA TAN-DOMINGO, M.D. (Authorization No. 115469–9W)

Please be advised that, by Consent Order (BPMC No. 08-114), effective June 27, 2008, the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), adopted Dr. Tan-Domingo's agreement wherein she admitted guilt to the charge of negligence on more than one occasion.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Tan-Domingo from the list of authorized physicians, effective July 7, 2008. Dr. Tan-Domingo's current address is 7564 Park Avenue, Lowville, New York 13367.

Dr. Tan-Domingo is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Tan-Domingo for treatment rendered prior to July 7, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Tan-Domingo regarding treatment she rendered prior to July 7, 2008, should not be denied due to her removal from the list of authorized physicians.
