Subject Numbers Regarding Health Provider Authorizations

MICHAEL CESAR, M.D. (Authorization No. 165509–1W)

Please be advised that, by Consent Agreement and Order (BPMC No. 08–75), of the New York State Department of Health, State Board for Professional Medical Conduct, effective May 21, 2008, Dr. Michael Cesar's license to practice medicine in New York State was suspended.

Pursuant to Workers' Compensation Law Section 13–d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Cesar from the list of authorized physicians, effective June 17, 2008. Dr. Cesar's current address is 5549 Hidden Pines Ct, Williamsville, New York 14221.

Dr. Cesar is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Cesar for treatment rendered prior to May 21, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross–examine Dr. Cesar regarding treatment he rendered prior to May 21, 2008, should not be denied due to the suspension of his medical license or his removal from the list of authorized physicians.

ALEXANDER FARCA, M.D. (Authorization No. 107267–7W)

Please be advised that, by Consent Order (BPMC No. 07–251), of the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective May 21, 2008, Dr. Alexander Farca surrendered his license to practice medicine in New York State or in any other jurisdiction where that practice is predicate on a valid New York State license to practice medicine.

Pursuant to Workers' Compensation Law Section 13–d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Farca from the list of authorized physicians, effective June 25, 2008. Dr. Farca's current address is 1 Lakeside Drive, Larchmont, New York 10538.

Dr. Farca is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Farca for treatment rendered prior to May 21, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross–examine Dr. Farca regarding treatment he rendered prior to May 21, 2008, should not be denied due to the surrender of his medical license or his removal from the list of authorized physicians.

ANTHONY NAPPI, M.D. (Authorization No. 115906–0W)

Please be advised that, by Surrender Order (BPMC No. 08–83) of the New York State Department of Health, State Board for Professional Medical Conduct, effective June 9, 2008, Dr. Anthony Nappi surrendered his license to practice medicine in New York State.

Pursuant to Workers' Compensation Law Section 13–d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Nappi from the list of authorized physicians, effective June 17, 2008. Dr. Nappi's current address is 35 South Hills Drive, New Hartford, New York 13413.

Dr. Nappi is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Nappi for treatment rendered prior to June 9, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross–examine Dr. Nappi regarding treatment he rendered prior to June 9, 2008, should not be denied due to the surrender of his medical license or his removal from the list of authorized physicians.

RICHARD YALDIZIAN, M.D. (Authorization No. 151575–8W)

Please be advised that, by Determination and Order (BPMC No. 08–76) of the New York State Department of Health, State Board for Professional Medical Conduct, effective May 23, 2008, Dr. Richard Yaldizian's license to practice medicine in New York State was revoked.

Pursuant to Workers' Compensation Law Section 13–d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Yaldizian from the list of authorized physicians, effective June 17, 2008. Dr.Yaldizian's current address is 16320 Crossbay Blvd., Howard Beach, New York 11414.

Dr. Yaldizian is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Yaldizian for treatment rendered prior to May 23, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross–examine Dr. Yaldizian regarding treatment he rendered prior to May 23, 2008, should not be denied due to the revocation of his medical license or his removal from the list of authorized physicians.
