MELCHIAS N. MUKENDI, M.D. (Authorization No. 187870-1W)

Please be advised that, by Determination and Order (BPMC No. 07-227) of the New York State Department of Health, State Board for Professional Medical Conduct, effective October 24, 2007, Dr. Melchias N. Mukendi's license to practice medicine in the State of New York was revoked.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Mukendi from the list of authorized physicians, effective November 5, 2007. Dr. Mukendi's current address is 61-15 7th Street, Apartment 6F, Rego Park, New York 11374.

Dr. Mukendi is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Mukendi for treatment or examinations rendered prior to October 24, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Mukendi regarding treatment or examinations he rendered prior to October 24, 2007, should not be denied due to the revocation of his license or his removal from the list of authorized physicians.

VENKATARAO KAMANI, M.D. (Authorization No. 219270-6W)

Please be advised that, by Consent Order (BPMC No. 07-217) of the New York State Department of Health, State Board for Professional Medical Conduct, effective October 11, 2007, Dr. Venkatarao Kamani was found to have engaged in professional misconduct and his license to practice medicine was suspended for at least thirty-six (36) months, stayed with probation for thirty-six (36) months.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Kamani from the list of authorized physicians, effective November 5, 2007. Dr. Kamani current address is 6108 Waitsfield Drive, Jamesville, New York 13078.

Dr. Kamani is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Kamani for treatment or examinations rendered prior to November 5, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Kamani regarding treatment or examinations he rendered prior to November 5, 2007, should not be denied due to his probation or his removal from the list of authorized physicians.
