TEMPORARY SUSPENSION — THOMAS F. MICELI, D.C.

Please be advised that, effective May 29, 2007, Thomas F. Miceli, D.C. consented to a temporary suspension of his authorization (Number C02908-4W) to treat workers' compensation claimants pursuant to Workers' Compensation Law §13-l(12). Dr. Miceli's business address is 33 Leeds Boulevard, Farmingville, New York 11738-1147. The suspension is effective until May 28, 2008.

While the suspension is in effect, Dr. Miceli is prohibited from rendering chiropractic treatment and care to individuals who have suffered work-related injuries. Medical reports submitted for treatment rendered prior to May 29, 2007 are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Miceli regarding treatment he rendered prior to May 29, 2007 should not be denied due to this suspension.

TEMPORARY SUSPENSION — MARC D. FELDERSTEIN, D.C.

Please be advised that, effective June 18, 2007, Marc D. Felderstein, D.C.'s authorization (Number C08253-9B) to treat or conduct independent medical examinations of workers' compensation claimants is temporarily suspended for one year pursuant to Workers' Compensation Law §13-l(12). Dr. Felderstein's business address is 3419 Quentin Road, Brooklyn, New York 11234. The suspension is effective until June 17, 2008.

While the suspension is in effect, Dr. Felderstein is prohibited from rendering chiropractic treatment and care to individuals who have suffered work-related injuries. Medical reports submitted for treatment rendered prior to June 18, 2007 are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Felderstein regarding treatment he rendered prior to June 18, 2007 should not be denied due to this suspension. In addition, testimony and/or depositions concerning independent medical examinations conducted through June 17, 2007 may be provided.
