VOLUNTARY RESIGNATION — CARMELO VILLA, M.D.

Please be advised that, effective December 6, 2006, pursuant to a Modified Consent Agreement and Order of the New York State Department of Health, State Board for Professional Medical Conduct (BPMC No. 01-261), Carmelo Villa, M.D.'s license to practice medicine was deemed inactive and he agreed never to register or reapply for a license to practice medicine in New York State.

Effective April 30, 2007, Dr. Villa voluntarily resigned his authorization (Number 125346-7W) to treat workers' compensation claimants. His current address is 1006 Bridge Mill Avenue, Canton, Georgia 30114.

Dr. Villa is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Villa for treatment or examinations rendered prior to December 6, 2006 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Villa regarding treatment or examinations he rendered prior to December 6, 2006 should not be affected by the inactive status of his medical license or this resignation.

PERMANENT REVOCATION — MOSHE HAIM OSTAD, M.D.

Please be advised that, by Determination and Order (BPMC No. 06-008), a Hearing Committee of the New York State Department of Health, State Board for Professional Medical Conduct, revoked Dr. Moshe Ostad's license to practice medicine in the State of New York, or in any other jurisdiction where that practice is predicated on a valid New York State license to practice medicine, effective January 25, 2006. Said revocation was temporarily stayed by the New York Supreme Court, Appellate Division, Third Department, which stay was later denied. The revocation went back into effect on March 13, 2006.

Effective May 7, 2007, Dr. Ostad's authorization (Number 113669-6W) to treat workers' compensation claimants was permanently revoked pursuant to Workers' Compensation Law §13-d(1). Dr. Ostad's current address is 65 Hickory Drive, Roslyn, New York 11576-2321.

Dr. Ostad is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Ostad for treatment or examinations rendered prior to March 13, 2006 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Ostad regarding treatment or examinations he rendered prior to March 13, 2006, should not be denied due to the revocation of his license or this revocation.

PERMANENT REVOCATION — HERON GEORGE RATTRAY, M.D.

Please be advised that, by Determination and Order (No. 05-265) of the Administrative Review Board for Professional Medical Conduct, New York State Department of Health, Dr. Heron Rattray's license to practice medicine in the State of New York, or in any other jurisdiction where that practice is predicated on a valid New York State license to practice medicine, was revoked, effective February 28, 2006.

Effective May 7, 2007, Dr. Rattray's authorization (Number 193948-7W) to treat workers' compensation claimants was permanently revoked pursuant to Workers' Compensation Law §13-d(1). Dr. Rattray's current address is 229 Lower Rock Point Road, Sound Beach, New York 11789.

Dr. Rattray is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Rattray for treatment or examinations rendered prior to February 28, 2006 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Rattray regarding treatment or examinations he rendered prior to February 28, 2006 should not be denied due to the revocation of his license or this revocation.
