RAPHAEL CILENTO, M.D. (Authorization No. 089966-6W)

Please be advised that, by Determination and Order (BPMC No. 07-43) of the New York State Department of Health, State Board for Professional Medical Conduct, effective March 1, 2007, Dr. Raphael Cilento's license to practice medicine in the State of New York was revoked.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Cilento from the list of authorized physicians, effective April 10, 2007. Dr. Cilento's current address is 436 Edgewood Avenue, Smithtown, New York 11787.

Dr. Cilento is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Cilento for treatment or examinations rendered prior to March 1, 2007 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Cilento regarding treatment or examinations he rendered prior to March 1, 2007 should not be denied due to the revocation of his license or his removal from the list of authorized physicians.

ESTRELLA M. MARTINEZ, M.D. (Authorization No. 182125-5B)

Please be advised that, by Consent Order (BPMC No. 06-199) of the New York State Department of Health, State Board for Professional Medical Conduct, effective August 29, 2006, Dr. Estrella Martinez' license to practice medicine in the State of New York was suspended for at least eighteen months.

Based on that action, pursuant to Workers' Compensation Law Section 13-d(4), the Chair of the Workers' Compensation Board temporarily suspended Dr. Martinez' authorization to treat and conduct independent medical examinations of individuals who have suffered work-related injuries or illnesses, effective October 19, 2006 through October 18, 2007. Pursuant to Workers' Compensation Law Section 13-d(5), the Chair has further exercised her authority to remove Dr. Martinez from the list of authorized physicians, effective April 10, 2007. Dr. Martinez' current address is 161 Sand Creek Road, Albany, New York 12205.

Dr. Martinez is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Martinez for treatment or examinations rendered prior to August 29, 2006 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Martinez regarding treatment or examinations she rendered prior to August 29, 2006 should not be denied due to the suspension of her license or her removal from the list of authorized physicians. In addition, testimony and/or depositions concerning independent medical examinations conducted through August 28, 2006 may be provided.
