Please be advised that, by Consent Order (Cal. No. 22801) of the New York State Board of Regents, dated October 24, 2006, Melvon C. Swanston, Ph.D.'s license to practice psychology in the State of New York, was, in part, suspended for one (1) month, from November 6, 2006 to December 5, 2006.

Effective March 30, 2007, Dr. Swanston voluntarily resigned his authorization (Number S10100-6B) to treat and conduct independent medical examinations of workers' compensation claimants. His current address is 764 South Drive, North Baldwin, New York 15510-1129.

During the actual suspension period from November 6, 2006 to December 5, 2006, and after March 29, 2007, Dr. Swanston is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Swanston for treatment or examinations rendered prior to November 6, 2006, and from December 6, 2006 to March 29, 2007, are valid, but are invalid for any treatment or examinations rendered from November 6, 2006 to December 5, 2006 and after March 29, 2007. Requests to cross-examine Dr. Swanston regarding treatment or examinations he rendered prior to November 6, 2006, and from December 6, 2006, to March 29, 2007, should not be denied due to the suspension of his license or this resignation. In addition, testimony and/or depositions concerning independent medical examinations conducted before November 6, 2006, and from December 6, 2006 to March 29, 2007, may be provided.
