Subject Numbers Regarding Health Provider Authorizations

Please be advised that effective April 10, 2006, Mark S. Wegerski, D.C., voluntarily resigned his authorization (number C05429-8B) to treat and to conduct independent medical examinations of workers' compensation claimants. Dr. Wegerski's last known office was located at 120 Cayuga Street #A, Fulton, New York 13069.

Dr. Wegerski is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Wegerski for treatment or examinations rendered prior to April 10, 2006, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Wegerski regarding treatment or examinations he rendered prior to the resignation's effective date should not be denied due to this resignation. In addition, testimony and/or depositions concerning independent medical examinations conducted through April 9, 2006, may be provided.
