Subject Numbers Regarding Health Provider Authorizations

Please be advised that effective February 3, 2006, the Workers' Compensation Board has revoked Mr. Albert DeSalvatore's license to act as a representative of claimants pursuant to Workers' Compensation Law §24-a and 12 NYCRR §302 et seq.

Mr. DeSalvatore may not solicit new clients, but is permitted to continue representing those clients for whom he is currently the representative of record until the close of business on February 2, 2006. After that date, claimants represented by Mr. DeSalvatore will need to find another representative, unless they choose to proceed with their claim without representation.
