Pursuant to Workers' Compensation Law Section 13-d(4), the Chair of the New York State Workers' Compensation Board has exercised her authority to temporarily suspend Dr. Fawzy Wasfy Salama’s authorization to treat injured workers within the New York State workers’ compensation system effective March 26, 2018. Dr. Salama's last known business address is:

The Chair’s designee’s investigation has revealed that the New York State Department of Health, State Board for Professional Medical Conduct (BPMC) has issued Order # 18-056, wherein Dr. Salama agreed not to contest the allegations in full satisfaction of the charges against him, including that in or about 2012, or in or about 2014, he failed to render appropriate care and treatment to multiple patients, and prescribed medications, including controlled substances, inappropriately to these patients. This resulted in a limitation on Dr. Salama’s license to preclude prescribing of controlled substances, with the exception of specific listed medications, until he successfully completes a Clinical Competency Assessment (CCA), to be reviewed by a Preceptor, which shall include but not be limited to the area of controlled substance prescribing, and a 36-month probationary period, which shall commence after his license limitation is stayed, during which he shall practice medicine only when monitored by a licensed physician, board certified in an appropriate specialty (“Practice Monitor”), and during which he must successfully complete a continuing education program as directed by the Office of Professional Medical Conduct.
Therefore, the Chair believes that Dr. Salama may be guilty of misconduct, and that such misconduct would detrimentally affect the quality of care provided to injured workers.
Dr. Salama is prohibited from rendering treatment and care to injured workers while his authorization is temporarily suspended. Reports submitted by Dr. Salama for services rendered prior to March 26, 2018, are valid but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Salama for services rendered prior to March 26, 2018, should not be denied due to the suspension of his authorization.
