The Workers' Compensation Board (Board) now requires authorized Acupuncturists, Chiropractors, Licensed Clinical Social Workers, Nurse Practitioners, Occupational Therapists, Physical Therapists, Physician Assistants, Physicians, Podiatrists and Psychologists to renew their authorization every two or three years. The renewal of a provider's authorization will coincide with their New York State Education Department license registration renewal. To streamline the process, the Board has transitioned to a fully electronic online renewal process through the Board's Medical Portal.

The authorization renewal process will ensure that our public directory of providers contains up-to-date information, enabling injured workers to easily identify and contact doctors available to treat. The Board's online Health Provider Search will include only those providers who are authorized.

Providers who fail to renew their authorization will:

  • Have their authorization rescinded (323.1[g] of 12 NYCRR).
  • Be removed from the Board's Health Care Provider and IME online search.
  • Be ineligible for the bill dispute resolution process if they were not authorized on the date of service.

  • Email (Use of Health Care Provider Emails)

  • Can you accommodate patients whose language is other than English?
  • Physician Assistants If your supervising physician changed, please enter the supervision end date for any physician listed that is no longer supervising you. If you are adding a supervising physician, attach a Supervising Physician Affirmation, signed by the Board authorized supervising physician.
  • Request authorization type change e.g., you are currently authorized to treat and now you want to apply to become authorized to perform Independent Medical Examinations as well (only applicable for a Physician, Chiropractor, Podiatrist or Psychologist).
  • Additional medical license(s)

Note: if you are a provider who holds multiple medical licenses, when you access the renewal form, the first license number you see may not be the license number you need to renew. Confirm all the information is correct for the first license (update as needed) and proceed through the form using the continue buttons until you get to the section about Another Medical License. Select the other medical license and review that information. Make any necessary changes and submit your form.

  • Have access to a computer or device with internet access. The Authorization Renewal form works best when accessed using Google Chrome.
  • Access the Medical Portal with your account. If you do not have a account, you must sign up for access
  • Complete and submit the WCB Authorization Renewal form
  • Attach PDF documents to form, if applicable, using the paper clip icon in the upper right corner of the form.
