Authorization No. 272372-4W
March 17, 2017
Pursuant to Workers' Compensation Law Section 13-d(4), the Chair of the New York State Workers' Compensation Board has exercised his authority to temporarily suspend Dr. Williams’ authorization to treat injured workers within the New York State workers’ compensation system effective March 13, 2017. Dr. Williams’ last known business address is One Atwell Road, Cooperstown, New York 13326.
The Chair’s designee’s investigation has revealed that the New York State Department of Health, State Board for Professional Medical Conduct (BPMC) has issued Order # 17-062, wherein Dr. Williams pled no contest to the First through Third Specifications, including that she failed to maintain appropriate medical records for patients, failed to appropriately document a patient’s diagnosis of depression by an oncologist after breast cancer diagnosis, failed to appropriately document a patient’s condition, other medications prescribed, and previous treatment of said patient, and failed to document the basis for an order of an MRI of a patient’s hip. This resulted in a Censure and Reprimand, and a 36-month probationary period, during which Dr. Williams may practice medicine only when monitored by a licensed physician, board certified in an appropriate specialty (“Practice Monitor”), and during which she must successfully complete a continuing medical education program in the area of medical record keeping.
Therefore, the Chair believes that Dr. Williams may be guilty of misconduct, and that such misconduct would detrimentally affect the quality of care provided to injured workers.
Dr. Williams is prohibited from rendering treatment and care to injured workers while her authorization is temporarily suspended. Reports submitted by Dr. Williams for services rendered prior to March 13, 2017 are valid, but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Williams for services rendered prior to March 13, 2017, should not be denied due to the suspension of her authorization.
This suspension remains in effect until further notice. Any questions regarding the authorized lists should be referred to the Medical Director’s Office at 1 (800) 781-2362.
