Subject Numbers Regarding Health Provider Authorizations

Please be advised that, effective September 1, 2009, the license to practice medicine of Kenneth Hopper, M.D., became inactive. No business address is available for Dr. Hopper.

Effective September 1, 2009, Dr. Hopper is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Hopper for treatment rendered or examinations performed prior to September 1, 2009, are valid, but are invalid for any treatment rendered or examinations performed on or after that date. Requests to cross-examine Dr. Hopper regarding treatment he rendered or examinations he performed prior to September 1, 2009, should not be denied due to the inactive status of his license to practice medicine.
