MAHENDER R. GORIGANTI, M.D. (Authorization No. 210930–4B)

Please be advised that, by Consent Order (BPMC No. 08–88) of the New York State Department of Health, State Board for Professional Medical Conduct, effective June 10, 2008, Dr. Mahender Goriganti was found to have engaged in professional misconduct.

Pursuant to Workers' Compensation Law Section 13–d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Goriganti from the list of authorized physicians, effective June 16, 2008. Dr. Goriganti's current address is 1000 East Genesee Street, Suite 100A, Syracuse, New York 13210.

Dr. Goriganti is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Goriganti for treatment or examinations rendered prior to June 10, 2008, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross–examine Dr. Goriganti regarding treatment or examinations he rendered prior to June 10, 2008, should not be denied due to his removal from the list of authorized physicians. In addition, testimony and/or depositions concerning independent medical examinations conducted through June 9, 2008, may be provided.

DESH DEEPAK SACHDEV, M.D. (Authorization No. 200885–2W)

Please be advised that, by Surrender Order (BPMC No. 08–87) of the New York State Department of Health, State Board for Professional Medical Conduct, effective June 10, 2008, Dr. Desh Deepak Sachdev surrendered his license to practice medicine in the State of New York or in any other jurisdiction where that practice is predicated on a valid New York State license to practice medicine.

Pursuant to Workers' Compensation Law Section 13–d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Sachdev from the list of authorized physicians, effective June 16, 2008. Dr. Sachdev's current address is 1137 Barnes Street, Franklin Square, New York 11010.

Dr. Sachdev is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Sachdev for treatment or examinations rendered prior to June 10, 2008, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross–examine Dr. Sachdev regarding treatment or examinations he rendered prior to June 10, 2008, should not be denied due to the surrender of his license or his removal from the list of authorized physicians.
