VOLUNTARY RESIGNATION – DR. RAFEAK MUHAMMAD, M.D.

Please be advised that effective May 10, 2005, Dr. Rafeak Muhammad voluntarily resigned his authorization (number 147499-8W) to treat workers' compensation claimants. Dr. Muhammad's office is located at 112-14 Liberty Avenue, Richmond Hill, NY 11419.

Dr. Muhammad is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted for treatment rendered prior to May 10, 2005 are valid, but are invalid for any treatment rendered after that date. Requests to cross-examine Dr. Muhammad regarding treatment he rendered prior to the resignation's effective date should not be denied due to this resignation.

VOLUNTARY RESIGNATION – DR. NORMAN MICHAEL HEYMAN, M.D.

Please be advised that effective September 16, 2005, Dr. Norman Michael Heyman voluntarily resigned his authorization (number 095786-0B) to conduct independent medical examinations of workers' compensation claimants. Dr. Heyman's offices are located at 245 Union Avenue, Bridgewater, NJ 08807-3064 and 77 Tarrytown Road, White Plains, NY 10607-1639.

Dr. Heyman is prohibited from conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted for examinations rendered prior to September 16, 2005 are valid, but are invalid for any examinations rendered after that date. Requests to cross-examine Dr. Heyman regarding examinations he rendered prior to the resignation's effective date should not be denied due to this resignation.
